Family Emergencies Absence Form

This form is to record information relating to time off for family emergencies and is to be completed by the employee and countersigned by their line manager.  It must be completed on every occasion of time off taken to deal with a family emergency.  It outlines all the information required by the employer.  Further details on the statutory family emergencies scheme can be found in the Parental Leave and Family Emergencies Policy.
